Abstract
Medicare Part D was implemented as a prescription drug benefit in 2006. Since then, high-cost new therapies have emerged, resulting in large increases in prices for pharmaceutical products, share of government spending from pharmaceutical products, and patient out of pocket costs.
Introduction
Since its implementation in 2006, Medicare Part D has seen increasing spending on drugs and higher out-of-pocket amounts for consumers, reaching a total $328.6 billion in 2016. 1 Many factors are at play, including the Anti-Kickback Statute “safe harbor”.
Methods
Building on prior work regarding the association of rebates and increased consumer costs in Medicare Part D, this study is structured as an ongoing case study of hepatitis C antivirals. Selected 2018 Medicare Part D formulary were compared by the frequency of various Hepatitis C treatments of different list prices, and presumably differing rebates.
Results
All of the drugs mentioned have the potential to cure a wide variety of patients depending on genotype, previous treatments and disease progression. The potential to cure HCV in the United States comes with wide ranging costs.
Discussion
Over the past three decades managed care organizations and plan sponsors have grown in popularity and product offerings have expanded. So too has the need to service their clients’ pharmacy and prescription medication coverage. PBMs were originally a cottage industry to which much of this work was outsourced.
Conclusion
The aforementioned policy proposals and regulatory actions are all steps in the right direction to bring transparency to the pricing and payment practices for high-cost drugs.
