To request a new Railroad Medicare PTAN for an individual provider or supplier (physician, nonphysician practitioner, ambulance, laboratory, ambulatory surgical center, etc.), enter the following:

  • PTAN assigned to the provider or supplier by the local Part B MAC
  • NPI linked to the PTAN the request is for
  • Last five digits of Tax Identification Number (TIN)

How to enter the provider's PTAN:
  1. If the PTAN is the individual number and begins with a 'P', then the provider would key *71 followed by the other numeric characters.
  2. If the PTAN is the group number and begins with 'CM', then the provider would key *23 *61 followed by the other numeric characters.
