Our Medicare plans include dental coverage, with either a yearly dental allowance or preventive dental with optional additional dental coverage. UCare Prime and UCare Aware both have a dental allowance.

Who is Medicare through?

What is UCare Connect?

UCare Connect + Medicare (HMO D-SNP) is a health plan that contracts with both Medicare and the Minnesota Medical Assistance (Medicaid) program to provide benefits of both programs to enrollees. Enrollment in UCare Connect + Medicare depends on contract renewal

You must continue to pay your Medicare Part B premium.

Members must use plan providers for routine care except for emergency care, out-of-area renal dialysis/urgent care, (and in-area urgent care when the UCare Medicare Group Plans or Group UCare for Seniors network is temporarily unavailable), and Point-of-Service benefits.If you obtain care from out-of-network providers neither Medicare or UCare Medicare Group Plans will be responsible for the costs.

UCare Connect is a Special Needs BasicCare (SNBC) program. It is available to individuals who are at least 18 years of age and under 65, have a certified disability, receive Medical Assistance (Medicaid) from the state with or without Medicare Parts A and B, and live in the service area

If your application status is “Incomplete”, call 1-888-243-8874 and speak with a UCare Enrollment Specialist. Or wait until you receive a call or letter from UCare with more explanation and instructions on how to proceed.

If UCare needs more information or documentation to complete your application, we’ll give you a call or send you a letter. Please follow the instructions you are given and respond by any deadlines you receive.

How to apply for Medicare for kidney transplant?

In order to be considered for Medicare coverage, you must show documentation from your doctor to having had a successful kidney transplant and/or that you don’t need regular dialysis anymore. If documents can’t be attached, please fax documentation to UCare at 1-866-283-43 41 or mail to UCare Enrollment, P.O. Box 52, Minneapolis, MN 55440. If you are not able to upload your documentation now, you can save your application without submitting and finish it later.
