When will the new UCARE Member ID card format be issued?
UCare introduced a new Member ID card format on Aug. 1, 2020. It will be issued for members who join UCare or seek a replacement card during the remainder of 2020. UCare will not replace cards issued earlier this year. Providers should accept both ID card designs through the end of 2020.
Which UCARE plans are not eligible for 2021 incentives?
UCare Basic, UCare Extended Basic and UCare $20/$50 Medicare Supplement members are not eligible for 2021 incentives. EssentiaCare Medicare plans: Grand, Secure and Access. UCare Medicare with M Health Fairview & North Memorial Health plans: Care Advantage and Care Core.
Are there any incentives for UCARE advocate Choice?
UCare Advocate Choice (HMO-I-SNP) and UCare Advocate Plus (HMO-I-SNP) members are not eligible for 2021 incentives. Members must have provider complete the voucher prior to returning it to UCare. Vouchers apply to current members, enrolled at the time of the exam, test, or screening. Limit one gift card reward per voucher per calendar year.

On Jan. 1, 2020, UCare changed the Payor ID to 55413 for UCare Medicare Plans, EssentiaCare and UCare Medicare with M Health Fairview & North Memorial claims submissions for 2020 dates of service. Some providers have been using the new Payor ID of 55413 for claims with dates of service in 2019.
