What is the minimum amount of nursing documentation required for a splint?
Occupational therapy for ADL self-care, splint/brace adjustment/training, etc. Skilled nursing documentation is required a minimum of every 24 hours.
Can you send ADR in one envelope?
This allows for several ADR response s to be sent in one envelope or package. Copies should be full sheets of one-sided paper. Do not include half sheets or double-sided paper. If your facility uses double-sided forms, verify that beneficiary's name is on each sheet of paper submitted.
Does Noridian accept a CD?
Noridian will only accept CDs/DVDs that are zipped using Adobe Security, WinZip, 7-Zip or Secure Zip. (Other formats will not be accepted and CD/DVD will be returned to provider) Images may be included in one Zip file or in multiple Zip files (for instance if one image per Zip is necessary).
How long does it take to change your address for a CMS ADR?

Fax ADR responses for all regions to 1-877-439-5479 (24 hours a day/7 days a week) Faxes should not exceed 200 pages. A copy of the ADR letter must accompany the fax.
Can I submit ADR electronically?
Providers can submit requested ADR documentation electronically through the esMD. This process will allow providers to submit medical documentation over secure electronic means. The process is secure, time efficient and cost effective!
