Reintervention after endovascular repair (EVR) of abdominal aortic aneurysms is common. However, the cumulative financial impact of reintervention after EVR on a national scale is poorly defined.

Methods
We studied patients who underwent EVR within the Vascular Quality Initiative registry ( www.vqi.org ). We linked patients in the registry to their respective Medicare claims file for long-term outcome assessment. 8 Patients were linked with 95.0% matching success.
Results
We included 1207 patients in the study ( Table ). All patients in the cohort had follow-up through 5 years or until death. The mean age was 76.2 years, and most were male (78.4%). Comorbid conditions were common, with 25.7% being active smokers, 35.2% having a history of coronary artery disease, and 26.7% being obese.
Discussion
This study highlights the cumulative costs incurred by Medicare associated with reinterventions among EVR patients and highlights the escalating costs associated with sequential procedures.
Conclusions
Each reintervention after EVR incurs a cost to Medicare that mirrors the cost of the index repair. Many EVR patients will undergo multiple reintervention procedures during their lifetime.

How Well It Works
When an aortic aneurysm is at risk of rupturing, or bursting open, the benefits of repairing the aneurysm can outweigh the risks. Repairing the aneurysm lowers the risk of rupture and can help a person live longer. Repairing a smaller aneurysm, which doesn't have as high a risk of rupture, does not help a person live longer.
Condition Information
Endovascular repair is less invasive than open surgery because it avoids a large incision in your abdomen or chest; only very small incisions are required. That means you are able to recover more quickly.
