Available at all family medicine clinics, our Patient-Centered Medical Home (PCMH) program is a holistic approach to health care that seeks to actively involve patients and families in their health. Our primary care providers, health coaches and nutritionists team with adult patients to manage the treatment of acute and chronic illness through preventative services and enhanced communication. The program aims to reduce hospital admissions and emergency department visits while improving the health of patients.
